POSITION SUMMARY:The LPN- Hospice cares for patients with terminal illnesses or other diagnoses, indicating they have fewer than six months to live. The LPN will provide end-of-life care to ensure patients are as comfortable as possible and have the highest possible quality of life. The LPN is typically part of a care team. This team collaborates to ensure individuals and their families have the physical, emotional, and spiritual support they require throughout the care process. KEY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS: - Provides patients with personalized care to improve their quality of life. - Offers mental, emotional, and physical support to patients and their families as they handle the end-of-life care process. - Provides patients with personalized care to improve their quality of life. - Offers mental, emotional, and physical support to patients and their families as they handle the end-of-life care process. - Tracks patients' symptoms, conditions, and behaviors to identify changes or signs of physical and mental decline. - Assist the Registered Nurse in carrying out the plan of care. - Assist the Registered Nurse in performing specialized procedures (as defined in the State Nurse Practice Act) as directed by the Registered Nurse under physician's orders. - Obtains physicians' orders for supplies, including hospital beds, oxygen, and medications for the patient - Help reinforce teaching to the patient/family members regarding end-of-life care. - Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences. - Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning. - Prepare clinical and progress notes as appropriate. - Keep the Case Manager informed of changes noted during visits. - Performs on-call duties (including holidays) per policy and agency need. - Actively participates in QAPI meetings and activities. - Adhere to Federal, state, and accreditation requirements, including Medicare and Medicaid regulations.   • Graduation from an accredited School of Practical Nursing. - One year or more of clinical experience in geriatrics; Hospice experience preferred. - Possess proficient assessment skills related to geriatric residents. - Fosters an environment honoring resident/patient preferences and choices. - Exhibits well-developed communication skills. Can professionally receive and follow oral instructions in English. Able to understand and use simple math. - Ability to cope with mental and emotional stress related to the position, function independently, and have flexibility and professionalism. - Very strong interpersonal skills and the ability to build relationships with all stakeholders. - Expert level written and verbal communication skills. - Demonstrate the highest level of customer/client service and response. - Work collaboratively in a team environment that promotes inclusiveness and communication among team members. CERTIFICATES, LICENSES, REGISTRATIONS
    • Graduation from an accredited School of Practical Nursing. - Must be licensed in the State of MISSOURI as a Licensed Practical Nurse - Current Basic Life Support (BLS) certification or ability to obtain before direct patient contact. - Valid driver's license and proof of insurance required. PHYSICAL DEMANDS: The physical demands described here represent those that an employee must meet to perform the essential functions of this job successfully. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions. - Required to drive from one location to another for services provided through the department in personal vehicle. - Occasionally required to carry or lift items weighing up to 10 pounds. - Occasionally required to carry or lift items weighing up to 25 pounds for short distances. Required assistance from another qualified employee when lifting objects over 25 pounds. - Occasionally required to stand or walk. - Frequently required to sit in a stationary position. - Occasionally performs activities that require stooping, bending and reaching, squatting, balancing, and trunk twisting throughout the day. - Constantly able to communicate verbally and in written form. - Frequently able to detect and discern both loud and soft sounds. - Constantly using hands and fingers to type, write, and file. - Frequently use near vision to view computer monitors and written materials. - Occasionally required to detect unusual odors that could indicate environmental issues. WORKING CONDITIONS: Category I: Position is Category I of Potential Exposure. Appropriate Personal Protective Equipment (PPE) is required for every employee in Category I tasks. Frequently exposed to blood, body fluids, fumes, airborne particles, and sharp objects potentially contaminated with blood-borne pathogens or other infectious agents. - The noise level in the work environment is usually moderate. - Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment. - Subject to frequent interruptions by staff, visitors, and volunteers. - Subject occasionally to hostile and emotionally upset staff and visitors.
