Additional Information About the Role
- Shift information: FT Day Shift (Monday - Friday 8 AM - 4:30
PM) - no on call, weekends, holidays or evenings
- Master Degree in a Human Services Field required
- Team dynamic: -3 Case Managers + Supervisor
- Location: Community Outreach; we cover the following counties
either in person or virtually: - St. Louis City, St Louis County,
Iron County, St. Francois County, Washington County and also have
clients in the Jefferson, Lincoln, and St. Charles counties as
well.
- MUST HAVE skills for this position: Must know sign language and
some knowledge of Deaf culture
- Job responsibilities: - Completing assessments, community
support such as linking clients to community resources with
housing, food assistance, etc.
- Working with clients of all ages in the communities face to
face or over Zoom; transporting clients in your own vehicle
Overview
BJC Behavioral Health is a community health center that provides
and coordinates behavioral health services for more than 8,000
seriously mentally ill adults and seriously emotionally disturbed
children in St. Louis City, St. Louis County, St. Fran--ois, Iron
and Washington counties. - As an Administrative Agent of the
Missouri Department of Mental Health (DMH), BJC Behavioral Health
serves as a major point of entry for people eligible for mental
health services funded by DMH and is responsible for serving as
gatekeeper to the public mental health system. - - -
Preferred Qualifications
Role PurposeProvide case management services to maximize
opportunities available to people living in the community who are
trying to recover from the catastrophic effects of a serious mental
illness. -Responsibilities
- Assesses service delivery needs of the client to determine
level of functioning, resources and needed services.
- Develops treatment plans that are individualized to the
patient's needs.
- Authorizes, coordinates, monitors, and adjusts services with
contract providers. Identifies, develops, and maintains good
relationships and linkages with a wide range of community
resources.
- Re-evaluates the services provided to ensure that outcomes are
met and that the level and type of services are still appropriate
to the need. Adjusts services accordingly.
- Uses American Sign Language (ASL) with speech/hearing impaired
clients.Minimum RequirementsEducation
- Master's Degree - Master's of Social WorkExperience

- Am Sign Language or equiv
